We will have ADULT
SUPERVISION all night. Parents are welcome to enter at anytime. No one under 18 Years of age will be allowed to leave the
rink unless
picked up by their own parent. Proper ID and signature will be required.
All skaters must return a signed
Coachlite permission slip which must be phone
verified before any skater will be allowed to enter Coachlite. If permission
slip
cannot be verified by a parent, admission will be denied to the skater.
NO EXCEPTIONS WILL BE MADE: ALL SKATERS UNDER 18 MUST HAVE A PERMISSION SLIP
SIGNED BY THEIR PARENT AND MUST BE VERIFIED BY COACHLITE PERSONNEL. All children under 8 years of age must be accompanied
by a parent. THIS IS TO VERIFY MY KNOWLEDGE AND PERMISSION FOR MY CHILD TO ATTEND THIS EVENT
